How to Apply

In order to apply for one of our BACR Phase IV Exercise Instructor training courses please submit the following:-

Two copies of the application form signed by your phase III endorser which can be downloaded below
One copy of your exercise qualifications
One copy of a character reference from your current employer
Two passport size photos
